An investigative consumer report will be or has been ordered from QiQ Research, Inc. (hereinafter “Agency”).
個人情報照会調査がQiQ Research, Inc. (以下「業者」と称する)に委任されました。
The investigative consumer report requested pursuant to your authorization below may contain information bearing on your character, general reputation, personal characteristics, mode of living, and credit standing. This information may be obtained from, but is not limited to, government or private agencies, civil and criminal records, and data including education, credit, and employment files, for a period of twelve (12) months from this date. During this background investigation, you understand that you may need to be contacted by your prospective employer or their agent for additional information pertaining to your investigative consumer report.
以下の委任により、「業者」は、性格、人物評価、個人的特長、生活状況、信用情報等の個人情報の照会調査を行います。今後12ヶ月間の間、公的機関、私的機関、民事・刑事訴訟記録、学歴記録、信用情報、職歴情報等から情報収集する可能性があります。この経歴調査の間、個人情報照会に際し、追加事前情報の為新規雇用者やその代理人等から問い合わせを受けることがあることをご了承ください。

A true copy of this Disclosure and Authorization shall be valid and have the same force and effect as the signed original.
情報開示と委任に関する署名された原文の真正なコピーは、原文と同等の効力があるものとする。
